We encourage you to preregister early in your pregnancy. There are three ways you can preregister:

By Mail   

Download and complete the preregistration admission forms listed below:

• Notice of Privacy Practices (NPP)
• Notice of Privacy Practices (NPP) Acknowledgement (2 forms)
• Consent for Treatment (2 forms)
• Conditions of Services (2 forms)
• Birth Certificate Worksheet
• Notice to Regional Perinatal Intensive Care Centers Patients (2 forms)

Please include a copy of your driver’s license and medical insurance card, front and back. 

You can mail your completed forms to:

Upon receiving your completed packet, your insurance information will be verified and your paper work will be processed. We will contact you if there are any concerns or questions regarding your paperwork.

In Person   

You may register in person at the Maternity Registration area, located on the first floor of St. Mary's just off of the main hospital entrance, Monday – Friday, 7:30a.m- 4:30p.m. Please bring your driver’s license and medical insurance card with you so that copies can be made for our files.
